IN COMPLETING THIS SUBMISSION FORM I UNDERSTAND AND AGREE TO ALL OF THE FOLLOWING TERMS:
Innovation World and their partner organizations will be known going forward in this Consent Form as Innovation World.
If I am registering myself, I confirm that I am 18 years old or older and I affirm that by completing this registration, I am giving INNOVATION WORLD the absolute right and permission for the use of my image and voice captured as part of these organized activities to be used by an authorized person acting for an authorized purpose of the aforementioned organizations on various print, broadcast and online platforms, including, but not limited to newspapers, television, radio, educational and news related websites, promotional literature and social media.
If I am registering a child under 18 years old, I confirm that I am the parent and/or legal guardian of the child or that I have the permission of the parent and/or legal guardian of the child and I affirm that by completing this registration, I am giving INNOVATION WORLD the absolute right and permission for the use of that child’s image and voice captured as part of these organized activities to be used by an authorized person acting for an authorized purpose of the aforementioned organizations on various print, broadcast and online platforms, including, but not limited to newspapers, television, radio, educational and news related websites, promotional literature and social media.
I agree that INNOVATION WORLD cannot accept responsibility for how the images, data or media recorded by third party organizations is used. I will not hold INNOVATION WORLD responsible for any deemed breeches in confidentiality or privacy that may arise as a consequence of recordings and/or posts made by any of these organizations.
