BY COMPLETING THIS FORM I AM AGREEING TO THE FOLLOWING:

Innovation World Photographic, Social Media & Video CONSENT FORM – Innovation World and their partner organizations, Hewitt Learning and InventFuture.Global will be known going forward in this Consent Form as The Innovation World.
If I am registering a child under 18 years old, I confirm that I am the parent and/or legal guardian of that child and I affirm that by completing this registration, I am giving The Innovation World the absolute right and permission for the use of that child’s image, content submitted and voice captures as part of these organized activities to be used by an authorized person acting for an authorized purpose of the aforementioned organizations on various print, broadcast and online platforms, including, but not limited to newspapers, television, radio, educational and news related websites, promotional literature and social media.

I agree that Innovation World cannot accept responsibility for images, data or media recorded subsequently accessed by outside organizations/non-partners, from the worldwide web from Innovation World websites or social media channels and I will not hold the Innovation World responsible for any deemed breeches in confidentiality or privacy that may arise from said breeches.
